An overview of our benefits package

We offer a comprehensive set of benefits to regular (non-temporary) employees who work a minimum of 24 hours per week. In general, benefits coverage starts on the first of the month following the date of hire. If an employee starts on the first of the month, then coverage begins immediately.

Health

Medical

Coverage is through Kaiser Permanente, and includes access to both Kaiser Permanente providers and an extensive array of “in-network” providers through three provider access networks: Access PPO, First Choice Health, and First Health. Premiums are covered at 100 percent for employees and dependents (age 0-20), and 50 percent for dependents (age 21-25) and partners/spouses.

The plan (Access PPO VisitsPlus Platinum LX 250) has a $250 per individual/$500 per family annual deductible and a $2,500 per individual/$5,000 per family annual out-of-pocket maximum.

Limited vision coverage is provided as part of the medical plan. Coverage includes a $100 annual allowance for glasses or contacts.

Dental

Coverage is through Delta Dental. Premiums are covered at 100 percent for employees and dependents, and 50 percent for partners/spouses. The plan covers 100 percent of diagnostic and preventative care, 80 percent of basic and restorative procedures, and 50 percent of major procedures. The plan has a $50 annual deductible and a $1,500 annual maximum benefit.

Paid Family and Medical Leave

Employees are eligible for 12 to 18 weeks of paid leave (up to $3,000 per week) in the event of a serious health condition; the birth, adoption, placement of a child; or to care for a family member who is experiencing a serious health condition.

Eligibility begins after an employee completes 340 hours of service.

Dependent Care Flexible Spending Account (FSA)

The Dependent Care Flexible Spending Account (FSA) allows employees to set aside up to $5,000 in pre-tax income to pay for out-of-pocket day care or dependent care expenses.
